Low-cost. You must complete an eligibility interview before your first visit.

Who can go

For low-income, uninsured individuals and families. Medicaid, Medicare, and Affordable Care Act plans all count as insurance, so they are for people who have none of those.

What to bring

Photo ID for each adult — a driver's license or state ID works even if it is EXPIRED, and they also accept a passport, visa, U.S. immigration documents, or a photo ID from another country. Plus proof of income (recent pay stubs, a tax return, a letter of support, or benefit documents) and proof of address (any piece of mail sent to you through the Postal Service). For children: a birth certificate.
